Every active USDOT number has to file an MCS-150 update every two years. That means every interstate carrier and every state-regulated intrastate carrier with a USDOT, no exceptions. Even if nothing about your operation changed, the FMCSA still wants the form.

When it's due
Every two years on a schedule FMCSA assigns based on the last two digits of your USDOT number. Each carrier has a specific month, and the update has to be in by the end of that month.
What happens if you miss it
If FMCSA doesn't get your biennial update, they mark your USDOT as inactive, sometimes within days of the deadline. Inactive means you cannot legally operate as a motor carrier until the update is in and the number is reinstated. Roadside stops, audits, and insurance lookups all flag an inactive USDOT immediately.
